Making a Claim
Please notify Eclipse Insurance Brokers of your claim as soon as possible. We will be able to assist you with any questions you may have and advise you on what to do next.

How to Download & Complete our online Claim Forms

Print out the claim form.
Answer all questions fully and honestly, a dishonest claim may result in you having no insurance cover.
If you have difficulty completing the form, please call us, we are more then happy to assist you complete these forms.
The policyholder must sign and date the claim form.
Post or Fax the completed claim form with any additional documentation to our office.
